Subjective Data Collection

Vital signs alone are a good indication of the patient’s health status, but personal patient feedback may identify underlying causes for the status that might have been missed. Subjective data collection allows you to identify personally reported information from your patients:

• How do you feel today?
• What did you eat for breakfast today?
• Did you take your medicine this morning?
• Is there swelling in your feet or ankles?
• Have you felt faint or light-headed today?

The collection of this type of information allows you to identify behavioral causes behind what is going on with your patient’s care.

EMR System

Cybernet Medical’s EMR system allows you to collect subjective data from patients using:

1) PALStar question and answer device

2) Questions available from the patient’s personalized login screen presented by the EMR system through the web

3) Interactive Voice Response (IVR) through the patient’s ordinary phone in two ways:

• Patient calls into a specific number, or
• The EMR system automatically calls the patient’s home phone at predetermined times with preset questions

Predefined structured dialog sets are available for your use covering CHF, COPD, Diabetes, Asthma, Obesity, Mental Illness, Hypertension, Quarantine, Wound Care, Cardiac Diagnosis, and other conditions. Unique to MedStar, you can easily create your own dialog sets.

Question sets can be set up with branching logic, allowing only applicable questions to be asked to the patient based on previous answers. This allows for more detail oriented questions to be asked when necessary, or skipped when not.
